Clinical Responsibilities for Care, Compliance, and Quality
As dynamics have changed in the post-acute care environment, SNFs are under the microscope of surveyors, auditors, and consumers, and documentation to support quality care is more important than ever.
From admission assessment to care planning and delivery to outcomes and discharges, the MDS and care plan can determine success or failure on various scales. As clinical teams struggle with the provision of care over paper, documentation serves as the communication link required for appropriate plans for care delivery to reimbursement. Facility documentation serves as a litmus test for solid policies, procedures, and organizational strengths. With the addition of PDPM and case-mix elements to the OBRA assessments, the MDS process requires review and evaluation before submission to third parties.
This deep dive will highlight:
- Core areas of the MDS and Care Planning
- Processes that facilities should consider for operational and organizational success
- Program will incorporate CMS, QIO and other readily available tools and formats for facilities to utilize in QA and QAPI programs.
