Marc N. Zimmet


Marc has been working exclusively in the SNF field for over 20 years.  His expertise is based on early experience preparing Medicare cost reports/appeals and assisting providers during the onset of the Medicare PPS demonstration project.  He now advises the firm’s diverse client base on navigating today’s complex regulatory/reimbursement landscape.  Marc is heavily involved in the next iteration of SNF reimbursement, assisting clients in positioning themselves within integrated delivery systems and properly “pricing” the care they provide under “fixed payment systems.”  Mr. Zimmet also advises on Medicare Advantage Special Needs Plans to assist SNFs in maximizing their participation in shared savings resulting from quality improvement.

Marc is nationally published on the topics of program integrity and service utilization under the RUGs payment system and served on a national task force established to determine the financial impact of case-mix refinement on SNFs.

He is an active member of many state and national industry associations, and lectures extensively on topics relating to SNF reimbursement and compliance.  Marc has conducted hundreds of educational programs on behalf of various state and national associations on matters relating to Medicare and Medicaid reimbursement.  He holds a degree in CPA Accounting from Syracuse University and an MBA in Healthcare Administration from Baruch College/Mount Sinai School of Medicine.

Sheryl Buchholtz-Rosenfield, RN, BC

Director of Clinical Operations

Sheryl has been active in the national SNF community for over 30 years.  She is a member of the Health Care Compliance Association’s Long Term Care Committee, working with representatives throughout the United States on protocols and policies effecting medical review and payment concerns. As a trainer of the MDS since 1990, she was a founder of AANAC, a national organization serving over 15,000 members associated with the assessment and care processes in skilled nursing facilities.

Sheryl has worked with the Centers for Medicare & Medicaid Services (CMS) on multiple projects, including serving on the Technical Expert Panel to evaluate the implementation and impact of the RUG PPS on SNFs.  She was a member of the clinical standards and training team for the federal DAVE project and served as a Data Monitor for the federal STRIVE project.  She also served as an MDS 3.0 Manual reviewer and continues to contribute to that project.

John L. Fazzio

Director of Financial Services

John L. Fazzio, Director of Financial Services for the past seventeen years, provides financial operational reimbursement support services to a variety of skilled nursing facilities nationwide.  Mr. Fazzio has provided billing consultation services to a number of long term care facilities with emphasis placed on the development and implementation for both Part A and Part B billing systems that meet the tailored requirements of the Prospective Payment System.

Prior to joining Zimmet Healthcare Services Group, LLC, for twenty-three years John was with United Healthcare, one of Medicare’s designated fiscal intermediaries.  As Manager of Medicare Audit & Reimbursement of United Healthcare for thirteen years, he supervised payment activity for over 225 participating providers. John was actively involved in all facets of Medicare reimbursement including audits, appeals, rate setting, and compliance issues.

John was also a leading participant in several Health Care Financing Administration task forces, whose functions were to improve, streamline, or design government reporting mechanisms, and to make recommendations for changing regulations. A graduate of Adelphi University, Garden City, NY, John has over forty-one years of Medicare reimbursement experience. 

Christopher Kennedy, MBA

Senior Reimbursement Analyst

Christopher Kennedy is a partner in the consulting firm of Zimmet Healthcare Services Group, LLC. Chris is the Senior Reimbursement Analyst and specializes in Medicare/Medicaid reimbursement topics, UB-04 accuracy, and MDS technical compliance for SNFs operating under Medicare PPS and Medicaid CMI payment systems.  Chris earned his MBA from Monmouth University and has been working in the post-acute industry since 1994.

Michael Sciacca, MPT, MHA

Director of Operations and Therapy Services

Michael is the Director of Therapy Services for Zimmet Healthcare Services Group, LLC. Michael specializes in strategy development and financial analysis for SNFs operating under Medicare PPS and Medicaid CMI payment systems.  Michael earned his degree in Physical Therapy from Quinnipiac University and has been a therapist for 10 years working the post-acute SNF industry.

Jake Komin, LNHA

Jake Komin is a licensed nursing home administrator and has extensive knowledge of both reimbursement and management of skilled nursing facility operations. He specializes in business development, customer relations, Medicare and Medicaid reimbursement, facility training, and skilled nursing facility operations. Prior to joining Zimmet Healthcare Services Group, Jake was Director of Reimbursement at Aristacare Health Services. In 2011 Jake founded Comprehensive Reimbursement Services (“CRS”), a reimbursement and compliance firm servicing over 50 clients. In 2013 CRS was acquired by Zhealthcare and Jake joined full time.  His strengths include 5 star analysis, compliance audits and ISNP development.
